Please accept this application for the Ole Miss Ladies Football Forum during the dates set forth. In consideration of your acceptance of the application, I will hereby release the University of Mississippi, Ole Miss Ladies Football Forum, and all its employees from all claims on account of any injuries which may be sustained while attending the Ole Miss Ladies Football Forum; and I agree to indemnify the University of Mississippi, Ole Miss Ladies Football Forum, and its employees for any claim which may hereafter be presented as a result of any such injuries. If medical attention is required for any injury or illness while in camp, I give permission for such medical care. This camp admits all qualified applicants without regard to race, color, religion or national origin. I certify the above information to be true, correct and complete. I furthermore give Ole Miss Ladies Football Forum permission to use my pictures and/or any recorded dialogue in conjunction with future promotion camp activities. BEFORE YOU REGISTER
Be sure to check out the "About Us" page to see the changes to the 2022 Forum. Because of the renovation to the Manning Center, the Forum will be a 4-hour event including breakfast and t-shirt.

RECIPES

We will not have a 2022 Cookbook. Recipes from the 2021 and the 2022 forum will be used in future forums. Click on the Recipe link at the top of the page to print your recipe forms. Bring your recipes to the forum and turn in at check-in.

REFUND POLICY
